In order to register, you will be required to provide a team name, division, and contact information, along with the full name and D.O.B of each member of your team. This form must only be submitted ONCE per team.

Teams must consist of at least 4 members, and are limited to 6 members.

FREE AGENT registration is also available below.

Each member is expected to print and sign the liability waiver linked below, along with the parental consent form for those under the age of 18. Please print and physically sign the forms. You may return these in-person prior to the start of your first game or send an image/scan of the completed form(s).
